The hospital specializes in both inpatient and outpatient procedures in Orthopedics, Joint Replacement, Spine, General, Bariatrics, Surgical Oncology, ENT, Reconstructive Surgery, and other surgical specialties.

Job Summary

The Case Manager PRN will be the leader in navigating the patient through the care coordination process. This process requires the case manager to have direct contact with the patient, their family, the patient's physicians, and the appropriate multidisciplinary health team members to ensure care coordination.

Responsibilities
  • Coordinate services to promote maximum outcomes in relation to appropriate length of stay, effective use of hospital resources, and established guidelines of care.
  • Facilitate interdisciplinary team collaboration and act as a liaison between the patient, physician, and payor in regards to care coordination.
  • Develop and maintain rapport with physicians and care team members.
  • Negotiate and resolve conflicts in a professional and assertive manner.
Requirements
  • Bachelor of Science in Nursing preferred with current license, State of Texas.
  • Minimum of 5 years acute hospital experience with a minimum of 3 years of utilization review, discharge planning, or case management preferred.
  • Knowledge and experience with medical terminology, InterQual or Milliman criteria, TJC and Federal/State regs, relative to discharge planning.
