We are seeking a skilled Registered Nurse to join our team at Lofland Park Center in Seaford, DE. As a Care Management Registered Nurse, you will play a vital role in providing high-quality patient care and collaborating with the nursing team to develop effective plans of care.
Key Responsibilities:- Perform comprehensive nursing assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and minimize rehospitalizations.
- Delegate patient care responsibilities to staff and supervise them to ensure tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure continuity of care.
- Graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- One to three years of experience working as a RN in a related setting is preferred.
- A dynamic and supportive work environment that fosters a culture of care and compassion.
- A competitive hourly rate of $40.00.
