Nurse Case Manager

Job Summary
We are seeking a highly motivated and organized Registered Nurse to join our team as a Peritoneal Dialysis Registered Nurse. As a Peritoneal Dialysis Registered Nurse, you will be responsible for providing education on peritoneal dialysis to patients and their care partners in a clinical setting, allowing them to dialyze from home.
Key Responsibilities
- Provide comprehensive patient care in an independent setting, applying core nursing skills to assess, diagnose, and implement complete care plans.
- Empower patients by teaching them to administer their own dialysis and take control of their health.
- Lead, coach, and guide patients, support systems, and care partners, working independently while building deep relationships.
- Manage patient cases, schedule home visits, and participate in on-call shifts, with flexibility to manage your own schedule.

Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 12 months of RN experience.
- Current driver's license, reliable transportation, and vehicle insurance in the state of residence.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing, or Bachelor of Science in Nursing (B.S.N), or three-year diploma from an accredited diploma program.
- Basic computer skills and proficiency in MS Word and Outlook.
Nice-to-Haves
- Previous experience in chronic hemodialysis, acute dialysis, home hemodialysis, or peritoneal dialysis nursing.
- Training, teaching, or healthcare education background.
- Previous case management experience.
- Familiarity with telehealth technologies.
- Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN), or Certified Peritoneal Dialysis Nurse (CPDN) certification.
