About the Role

We are seeking a skilled RN Case Manager to join our team at Nuvance Health. As a RN Case Manager, you will play a critical role in providing high-quality patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and socio-economic needs, and develop individualized plans of care to address these needs.
  • Provide direct skilled nursing care to patients in accordance with physician orders and agency policies.
  • Collaborate with patients, families, caregivers, and healthcare providers to ensure seamless care coordination and communication.
  • Participate in quality assurance and performance improvement initiatives to drive continuous quality improvement.
  • Supervise and provide clinical direction to LPNs as needed.
Requirements

To be successful in this role, you will need:

  • A strong background in nursing, with a focus on home health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong research and problem-solving skills, with the ability to analyze complex patient data and develop effective care plans.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
Working Conditions

This role requires:

  • Manual dexterity and ability to lift up to 50 pounds.
  • Occasional exposure to dirt, odors, and other environmental hazards.
