We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Extendicare Home Health of West Tennessee. As a LPN, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.
Responsibilities:- Provide quality patient care and perform technical skilled care in the patient's home, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Instruct the patient's family in the care of the patient and maintenance of a healthy environment for the patient.
- Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
