We are seeking a highly skilled and compassionate Physician Access Services Coordinator to join our team at the Mount Sinai Health System. As a key member of our healthcare team, you will play a vital role in facilitating the provision of hospital services to affiliated practices and institutions.

Key Responsibilities
  1. Serve as a Liaison: Establish and maintain effective relationships with referring physicians, patients, and Mount Sinai Health System departments to ensure seamless communication and coordination of care.
  2. Provide Exceptional Customer Service: Demonstrate exemplary customer service skills while interacting with patients, physicians, and other stakeholders to ensure a positive and respectful experience.
  3. Coordinate Patient Care: Responsible for the complete and accurate registration of all patients, ensuring timely and efficient processing of referrals and appointments.
  4. Communicate Effectively: Promptly answer and screen inquiries, adhering to confidentiality agreements, policies, and procedures.
  5. Facilitate Referrals: Coordinate referrals to the appropriate Mount Sinai Health System provider, set up necessary appointments, and communicate with referral sources, patients, and authorized family members.
  6. Follow Up and Follow Through: Promptly follow up via email and/or call with referred providers and specialty departments within the health system to confirm appointment scheduling and ensure timely follow-through.
  7. Advocate for Patients: Advocate for patients, families, and referring physicians to promote overall satisfaction with accessing and utilizing Mount Sinai Health System services.
  8. Support Specific Population Needs: Provide support for specific population needs as required by the program or department.
  9. Document Patient Encounters: Document all patient encounters and enter all data into referral tracking systems.
  10. Comply with Policies and Procedures: Follow all policies, protocols, and procedures to ensure referral requests are handled appropriately.
Qualifications

We are seeking a candidate with a Bachelor's Degree or equivalent in education, training, and experience, as well as 3 years of related experience in a healthcare setting. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
