Job Summary

We are seeking a skilled Care Manager to join our team at Tammy Lynn Center (TLC). As a Care Manager, you will be responsible for promoting integrated health services and coordinating care for individuals with complex needs.

About the Role

The ideal candidate will have a bachelor's degree in a related field and at least two years of experience working directly with individuals with intellectual or developmental disabilities (I/DD).

Responsibilities include:

  • Providing comprehensive care management services, including person-centered planning and outreach activities;
  • Conducting comprehensive assessments and reassessments within required timeframes;
  • Facilitating the completion of care plans and ISP documents;
  • Engaging extended care teams and providers in care plan development and implementation;
  • Utilizing clinical consultants for expertise in planning and care reviews;
  • Supervising Care Extenders and their assigned care management activities;
  • Completing all other relevant responsibilities as assigned by the supervisor.
Key Qualifications

To succeed in this role, you will need:

  • A bachelor's degree from an accredited college or university in a field related to health, psychology, sociology, social work, nursing, or another relevant human service area;
  • Licensure as a registered nurse (RN);
  • Two years of experience working directly with individuals with I/DD; and
  • For care managers serving members with long-term services and supports (LTSS) needs, two years of prior LTSS and/or home and community-based services (HCBS) coordination, care delivery monitoring, and care management experience.

This is a full-time regular position with a flexible schedule that may include rotation in on-call coverage. The estimated annual salary for this role is $62,000-$80,000, depending on qualifications and experience.
