Job Summary

In conjunction with the Head Nurse, this LPN will provide first-line planning, coordination, coaching, and organization of daily operations to ensure quality patient care in the assigned practice group.

Responsibilities include providing direct nursing care to Health Center patients as needed, based on patient demand and staff capacity. The LPN will function under the supervision of the Head Nurse, accepting guidance from the Associate Medical Director, Physicians, Nurse Practitioners, and/or Physician's Assistants.

Key Responsibilities
  • Plan, direct, coach, problem-solve, and supervise the day-to-day work of clinical support staff.
  • Troubleshoot staff and patient questions and problems as necessary, documenting and communicating issues and resolutions to the Head Nurse on a regular basis.
  • Work closely with the Head Nurse to assess systems and suggest improvements in systems and processes.
  • Maintain a cooperative relationship with all healthcare providers, Operations Manager, and Business Office Supervisor to determine appropriate patient appointments and scheduling needs.
  • Provide feedback and input to the Director of Patient Services and Head Nurse on performance and training needs of support staff.
  • Monitor patient and clinical support staff scheduling, ensuring adequate services and staff coverage at all times.
  • Monitor patient flow and assist in preparation of patients for exam, including taking of vital signs and presenting problems.
  • Perform related work as assigned and required to meet program needs, filling in for clinical positions as needed in the planned and unplanned absence of clinical support staff.
  • Lead daily clinical staff huddles and participate in routine clinical and monthly total staff meetings, meeting routinely with the Head Nurse to ensure coordination and effective communication.
Requirements
  • Current New York State Practical Nurse's License.
  • 7 years of experience as an LPN, a portion of which must be in ambulatory care; supervisory experience preferred.
