About the Role

The Pharmacist Per Diem is a key member of our healthcare team, responsible for maintaining pharmaceutical dispensing functions, including clinical review, in accordance with all legal and regulatory requirements. This role provides pharmaceutical care and products to all patients, ensuring safe and effective medication therapies.

Key Responsibilities
  • Interpret and evaluate medication orders for appropriateness of therapy, incompatibilities, dosage, and age, and transcribe to computerized patient medication profiles.
  • Seek clarification on incomplete medication orders and obtain indications.
  • Dispense pharmaceuticals and controlled substances to patient care areas in a safe and timely manner.
  • Maintain the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Provide direction to pharmacy staff and ensure effective operation of the pharmacy department.
  • Verify daily activities of pharmacy technicians, interns, and residents.
  • Ensure safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
  • Detect and report suspected adverse drug reactions and medication occurrences.
  • Utilize formulary initiatives and clinical review to provide adequate pain management for patients.
  • Utilize principles of drug dosing in pediatric patients to provide medications to patients in correct doses and formulations.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
  • Adjust dosages or choice of medications based on decreased renal clearances in the elderly or other patients with compromised renal function.
  • Review and interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.
  • Participate in the review of opiate medications that are prescribed for appropriateness.
  • Read, extract, and interpret information in patient charts.
  • Provide clinical consultations and clarification to practitioners and suggest appropriate cost-effective therapeutic alternatives.
  • Provide drug information to professional staff, patients, and families.
  • Perform initial screening of order entry to provide information for interventions and document all clinical activities and interventions.
  • Monitor provision of off-hours pharmacy operations, stock medications, controlled substances, medications brought from home, and patient education.
  • Conduct regular rounds on nursing units to assess compliance with requirements for medication control and storage.
  • Participate in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs.
  • Perform monthly and biennial controlled substance audits.
  • Assist with pharmacokinetics consult service and drug dosing per hospital protocol.
  • Prepare annual evaluation of the Medication Management program, annual review and approval of the hospital formulary, as well as lists of Look Alike Sound Alike Medications and High Alert Medications.
Requirements
  • Graduate degree from an accredited School of Pharmacy. Doctor of Pharmacy (PhD) degree preferred.
  • Minimum experience: hospital internship rotation.
  • Active and in good-standing state Pharmacy license.
  • Certification by the National Certification Board for Pharmacists and any required licensure.
  • CPR and de-escalation/restraint certification may be required based on state requirements or facility policy.
  • First aid may be required based on state or facility requirements.
