Key Responsibilities:- Provide exceptional patient care, focusing on prevention, early intervention, and management of chronic conditions.
- Lead a care team to deliver VIP patient care, working towards targets and metrics.
- Partner with patients to set and achieve health goals, using evidence-based medicine and clinical, nutritional, and lifestyle-based interventions.
- Stay current on best practices for improving patient outcomes and operations, influencing patients, teams, and yourself to achieve the best results.
- Collaborate with hospitalists, managers, and medical directors to ensure compliance with guidelines and quality management programs.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty (Board Eligibility required).
- Basic Life Support (BLS) certification and current DEA number for schedule II-V controlled substances.
- Bilingual in English/Spanish required.
