We're seeking a highly skilled and compassionate Primary Care Physician to join our team at IntuneHealth, a part of ChenMed. As a Physician Leader in Senior Care, you will be responsible for delivering high-quality, patient-centered care to our Medicare Advantage seniors.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, with a focus on geriatric medicine.
- Take full accountability for patient care and outcomes, seeking consultation from specialists when needed.
- Work closely with our care teams to ensure compliance with guidelines and quality measures.
- Engage with patients and their families to educate them on healthy lifestyle choices and disease management.
- Participate in risk and quality management programs, clinical meetings, and other initiatives to promote patient health and company goals.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (Board Eligibility is required).
- Basic Life Support (BLS) certification.
- Current DEA number for schedule II-V controlled substances.
