Job Summary:

The Case Manager Therapist will work closely with patients, families, and healthcare teams to develop and implement personalized treatment plans. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Develop and implement individualized treatment plans for patients
  • Conduct thorough assessments and evaluations to identify patient needs
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Maintain accurate and timely documentation of patient progress

Requirements:

To be considered for this role, you must have a Master's degree in a relevant field, such as psychology or social work. One year of experience in case management or a related field is also required. Advanced licensure (LMFT, LPC, LCSW) or certification (NCC/LSW) is preferred.
