Key Responsibilities:

  • Patient Outreach: You will engage with patients and their caregivers to identify their needs and develop personalized care plans.
  • Care Coordination: You will collaborate with healthcare providers and other stakeholders to ensure seamless care transitions.
  • Data Management: You will maintain accurate records and reports to track patient progress and program outcomes.
  • Project Support: You will assist with departmental initiatives and projects as needed.

Requirements:

  • A high school diploma or equivalent certificate
  • At least six months of experience in an administrative and customer service role in a healthcare setting
  • Bilingual: English/Spanish speaker (preferred)
  • Strong organizational skills and ability to manage multiple projects and teams
  • Excellent communication and problem-solving skills

Estimated Salary Range: $21.84 - $33.26 Hourly
