Job SummaryWe are seeking a skilled Case Manager to join our team. As a Case Manager, you will provide support and structure to residents in developing program plans, controlling and securing facility and residents, and operating within legal constraints and contract agency stipulations.
Key Responsibilities- Prepare written personalized programs for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals.
- Review plans with residents on a regular basis and document progress and/or revisions.
- Assist residents in establishing personal budgets, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities.
- Monitor facility sanitation and safety, and assign daily housekeeping chores to residents to comply with facility standards.
- Verify residents' jobs to ensure employment is suitable under established program plans, and counsel with unemployed residents to provide guidance in obtaining employment.
- Verify residents' movement according to contract, and prepare terminal/discharge reports upon resident release to summarize program performance and satisfy contract requirements.
- High school diploma or GED.
- One (1) year experience as Monitor I with or two (2) years in directly related job experience (case management, counseling, therapeutic environment).
- Associates or Bachelor's degree will substitute for experience.
- Familiar with community corrections systems and current community corrections practices.
- Ability to effectively interface with residents and staff, supervise residents, and react quickly and appropriately in an emergency situation.
