Job Description:

We are seeking an experienced Clinical Nurse Specialist Fellow to join our team. This role will involve providing expert population-based, family-centered clinical care and consultation within their area of expertise to improve outcomes and promote integrated healthcare. The successful candidate will be part of a centralized CNS team, working across the organization in interdisciplinary teams.

Responsibilities:
  • Manages the care of complex and/or vulnerable populations through expert direct care, care coordination, and collaboration with patients and families, as well as other members of the healthcare team.
  • Demonstrates a commitment to patient safety by being knowledgeable about evidence and best practices for safety, speaking up about safety issues, and changing practice to enhance safety.
  • Shares errors or potential errors through system reporting mechanisms and communicates these with appropriate leadership.
  • Demonstrates shared accountability for safe practice.
  • Supports other members of the healthcare team, including nurses, physicians, and allied health professionals, through collaborative clinical practice, education, research/evidence-based practice, and consultation.
