Nurse-Family Partnership Public Health Nurse Home Visitor

Job Summary:

We are seeking a compassionate and motivated Public Health Nurse Home Visitor to join our team at Public Health Management Corporation. As a Nurse-Family Partnership Public Health Nurse Home Visitor, you will be responsible for providing comprehensive nursing services to first-time pregnant individuals and their families in Philadelphia.

Key Responsibilities:

  • Provide regularly scheduled in-home visits and telehealth visits with clients.
  • Build and maintain a caseload of 25 active clients within nine months of employment.
  • Develop therapeutic relationships with clients utilizing concepts and processes from motivational interviewing, reflective listing, and Facilitating Attuned Interactions.
  • Plan home visits in accordance with client goals and Nurse-Family Partnership outcomes.
  • Support the achievement of client-identified goals through one-on-one education, active listening, screening, and referral processes to community resources.
  • Document all work appropriately and in a timely fashion using the Apricot and Penelope (Flo) Case Management Platforms.
  • Uphold the policies, procedures, guidelines, and standards of Nurse-Family Partnership and Public Health Management Corporation.
  • Adhere to American Nurses Association and Pennsylvania nursing process and standards, code of ethics, HIPAA confidentiality standards, and the Nurse-Family Partnership model of home visitation.
  • Contribute to and participate in virtual and on-site clinical case conferences and professional development meetings.
  • Meet weekly with the Nurse-Family Partnership supervisor for clinical reflective supervision and utilize the reflective process in supervisory sessions.
  • Utilize nursing process to assess, plan, and document physical, emotional, social, and environmental needs of clients and their families as they relate to the Nurse-Family Partnership domains of care.
  • Schedule joint home visits with Nurse-Family Partnership Supervisor every four months (three times a year) to support professional development.
  • Consult and collaborate with other professionals involved in providing services to clients and their families.
  • Collaborate with Public Health Management Corporation outreach staff regarding community sources and disposition of referrals to the Nurse-Family Partnership program.
  • Contribute to a positive work environment that enables all staff to function at their highest potential.
  • Provide timely documentation to data entry staff for input.
  • Review documentation and data system reports entered in state, local, and national databases for accuracy, uniformity, and completeness.

Requirements:

  • Current Pennsylvania nursing licensure in good standing.
  • Work or student experience in public health, maternal-child health, mental health/behavioral health, or related field such as Early Childhood services in community settings.
  • Baccalaureate Degree in Nursing from Accredited University.
  • Current CPR/BLS certification.

Work Environment:

This position requires a flexible schedule, including some early evenings and weekends, with consistent availability from 9 AM to 5 PM weekdays. Onsite attendance several days each week for the first 3-4 weeks of employment is required. Work within the County/City of Philadelphia boundaries, including all city zip codes, is necessary. Physical ability to perform job in home and community environments is required. Light to moderate lifting and transport of program/client materials is also required. Use of personal automobile during expected work hours is necessary. Travel throughout designated area to multiple locations, including government and private agencies and partners as related to work assignment, is required. Prolonged computer-related exposure, including ability to sit and stand at workstations for extended periods of time, is also required. Local to regional travel or as required by NSO and funders is necessary.

Salary:

Pay Grade: 22. Based on qualifications and experience.
