Job Title: Public Health Nurse Home Visitor

Public Health Management Corporation (PHMC) is a leader in public health, and we are seeking a compassionate and motivated Public Health Nurse Home Visitor to join our team. As a Public Health Nurse Home Visitor, you will be responsible for providing comprehensive nursing services to first-time pregnant individuals and their families eligible for the Nurse-Family Partnership (NFP) Program in Philadelphia.

Responsibilities:
  • Provide regularly scheduled in-home visits and telehealth visits with clients.
  • Build and maintain a caseload of 25 active clients within nine months of employment.
  • Develop therapeutic relationships with clients utilizing concepts and processes from motivational interviewing, reflective listing, and Facilitating Attuned Interactions (FAN).
  • Plan home visits in accordance with client goals and NFP outcomes.
  • Support the achievement of client-identified goals through one-on-one education, active listening, screening, and referral processes to community resources.
  • Document all work appropriately and in a timely fashion using the Apricot and Penelope (Flo) Case Management Platforms.
Requirements:
  • Current Pennsylvania nursing licensure in good standing.
  • Work or student experience in public health, maternal-child health, mental health/behavioral health, or related field such as Early Childhood services in community settings.
  • Baccalaureate Degree in Nursing from Accredited University.
  • Current CPR/BLS certification.
Work Environment:

The Public Health Nurse Home Visitor will work within the County/City of Philadelphia boundaries, including all city zip codes. The position requires a flexible schedule, including some early evenings and weekends, with consistent availability from 9 AM to 5 PM weekdays. Onsite attendance several days each week for the first 3-4 weeks of employment is required.

Salary:

Pay Grade: 22. Based on qualifications and experience.
