About This Role
We are seeking a skilled Registered Nurse to join our team at Prisma Health's Greer Medical Campus. As a Medical Professional in Inpatient Care Coordination, you will be responsible for providing high-quality patient care and coordinating with healthcare teams to ensure seamless transitions between levels of care.
Job Summary
This is a non-management role that requires a strong clinical background and excellent communication skills. You will work collaboratively with providers and other members of the healthcare team to develop and implement individualized plans of care for patients. Your primary focus will be on delivering evidence-based practices and promoting patient-centered care in an acute inpatient setting.
Key Responsibilities
- You will collect and analyze data to complete comprehensive nursing assessments, identifying subtle or actual changes in patients' status and responding accordingly.
- You will collaborate with healthcare teams to develop and update individual plans of care, ensuring that priorities align with patient needs and goals.
- You will provide direct care to assigned patients, delegating tasks as needed and ensuring the quality of care delivered.
- You will monitor and evaluate patient responses to interventions, revising plans of care as necessary to achieve optimal outcomes.
- You will serve as a patient advocate and coordinator of care, working collaboratively with healthcare teams to ensure continuity and coordination throughout the continuum of care.
Requirements
- A minimum of one (1) year of experience as a Registered Nurse in an acute care setting is required. Alternatively, successful completion of the Prisma Health Nurse Residency Program or a comparable residency program from another organization may be considered in lieu of this requirement.
- Associate degree in Nursing; Bachelor's degree in Nursing preferred.
Work Environment
This role is based at Prisma Health's Greer Medical Campus, which offers a dynamic and supportive work environment. As a member of our team, you will have opportunities to grow professionally and contribute to the delivery of exceptional patient care.
Estimated Salary Range: $65,000 - $85,000 per year
