Job Title: PRN Infusion Nurse Practitioner

AleraCare, a leading provider of ambulatory infusion treatments and specialty pharmacy services, is seeking a skilled Nurse Practitioner to join our team in Yuma, AZ.

About the Role:

We are looking for a highly motivated and experienced Nurse Practitioner to provide high-quality patient care in our infusion center. As a PRN Nurse Practitioner, you will be responsible for delivering nursing services to assigned patients in accordance with their individualized plan of care.

Responsibilities:
  • Maintain patient confidentiality and function as a patient advocate, treating patients and families with respect and dignity.
  • Perform initial patient evaluations/assessments and develop a written plan of care.
  • Prepare clinical and progress notes, coordinate services, and inform the physician and other personnel of changes in the patient's condition and needs.
  • Responsible for the clinical record of each patient receiving nursing care, ensuring documentation meets all standards and policies.
  • Collaborate with physicians and other members of the healthcare team to provide optimal care for patients.
  • Evaluate and reevaluate patient needs, providing patient and/or family with health education, counseling, and referrals to other healthcare professionals and/or community resources when appropriate.
  • Order and/or interpret lab work and diagnostic tests according to patient need and/or plan.
  • Demonstrate the ability to prescribe medications and non-pharmacological therapies based on patient diagnoses, health history, and age.
  • Participate in the company's Performance Improvement and Continuous Quality Improvement (CQI) programs.
  • Assume responsibility/ownership of clinic location(s) and support a culture of safety and quality care.
Requirements:
  • Masters Degree from an accredited Nurse Practitioner Program.
  • Holds a current NP state license in the state of practice.
  • At least 4-5 years of advanced practice registered nursing (APRN) experience, ambulatory and infusion nursing experience is preferred.
  • Strong interpersonal, time management, and organizational skills.
  • Strong written and verbal communication skills.
  • Experience with electronic medical records (EMR).
  • Ability to recognize and direct information to the appropriate healthcare provider.
  • Computer skills that include word processing and efficient use of the internet and e-mail.
  • Current CPR card, advanced practice certification, current APRN licensure, valid drivers license, reliable transportation, automobile insurance.
