AleraCare is seeking a skilled Nurse Practitioner to join our team on a per diem basis in our Yuma, AZ infusion clinic. As a key member of our healthcare team, you will provide high-quality nursing services to patients in accordance with their individualized plans of care.
Responsibilities- Conduct initial patient evaluations and assessments, developing a written plan of care that addresses the patient's unique needs.
- Regularly reassess patient conditions and update plans of care as necessary.
- Provide preventive nursing procedures and initiate appropriate interventions to promote patient well-being.
- Counsel patients and families on meeting nursing and related needs, providing education and support as needed.
- Collaborate with healthcare team members to ensure seamless care coordination and communication.
- Participate in performance improvement initiatives and contribute to the development of policies and procedures.
- Graduate of an accredited school of nursing and current NP state license.
- At least one year of nursing experience, with infusion nursing experience preferred.
- Effective interpersonal, time management, and organizational skills.
- Strong communication skills in speaking and writing.
- Ability to recognize and direct information to the appropriate healthcare provider.
- Computer skills that include word processing and efficient use of the internet and email.
