Job Summary

CVS Health is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better health outcomes.

Key Responsibilities
  • Patient-Centered Quality and Safety
    • Utilize your expertise in medical conditions, risk factors, and pathophysiology to provide evidence-based care to patients.
    • Conduct thorough history and physical assessments, accurately documenting patient information in electronic health records.
    • Provide patient counseling on health maintenance, disease prevention, and treatment options.
    • Evaluate and treat patients' medical needs with safe and high-quality treatment.
    • Collaborate with the clinical care team to ensure seamless patient care and coordination.
  • Communication and Interdisciplinary Collaboration
    • Work effectively with the clinical care team to deliver high-quality, patient-centered care.
    • Participate in clinical care team huddles and contribute to the development of care plans.
    • Communicate effectively with patients, families, and healthcare professionals to ensure optimal patient outcomes.
    • Adapt to changing patient needs and priorities, demonstrating flexibility and a patient-centered approach.
  • Healthcare Environment Management
    • Manage clinical and non-clinical duties efficiently, prioritizing tasks to ensure seamless patient care.
    • Collaborate with the clinical care team to enhance operational effectiveness and promote cost containment.
    • Develop and maintain positive relationships with patients, families, and healthcare professionals.
    • Stay up-to-date with industry developments, best practices, and regulatory requirements.
  • Business Acumen
    • Contribute to the development and implementation of business plans to meet identified goals and objectives.
    • Monitor and analyze key performance indicators to inform care delivery and quality improvement initiatives.
  • Autonomy
    • Work independently, prioritizing tasks and solving problems to ensure seamless patient care.
    • Take initiative to meet identified learning needs, staying current with industry developments and best practices.
    • Collaborate with the clinical care team to promote a culture of excellence and continuous improvement.
Requirements
  • Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
  • A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
  • Current National Board Certification in Family Practice.

  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
  • Self-motivated, prioritizing and solving problems, taking initiative, and advocating for patients and the practice.
  • Effective verbal, written, and electronic communication skills.
  • Outstanding organizational skills and ability to multi-task.
  • Initiative, problem-solving ability, adaptability, and flexibility.
  • Ability to work remotely in a clinical care team culture.
  • Ability to work without direct supervision and practice autonomously.
  • Proficient with information management and technology.
  • Willingness to obtain multi-state licensures.
  • Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
