Career Overview:

We are seeking a skilled Home Health Care Manager to join our team at Cantex. As a Home Health Care Manager, you will be responsible for supervising and managing all aspects of patient care, including LVN and HHA supervision.

Job Summary:

The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to read, write, and speak English fluently. You will work closely with the physician, clinical team, and patient to ensure a smooth transition from hospital to home care.

Key Responsibilities:

  • Supervise and manage all aspects of patient care, including LVN and HHA supervision
  • Coordinate visits among all disciplines (SN, PT, OT, ST, MSW, HHA)
  • Initiate referrals to appropriate ancillary services
  • Facilitate interdisciplinary communication to ensure optimal service delivery and patient progress
  • Lead weekly interdisciplinary case conferences for patient caseload
  • Establish specific social action plans and realistic goals for each patient
  • Coordinate discharge planning with patients, clinical team, and agency

Requirements:

  • Registered Nurse, licensed per state requirements
  • 2 years of experience in Home Health preferred
  • Good communication skills, assessment skills, and strong clinical documentation skills
  • Ability to read, write, and speak English fluently
  • Reliable transportation and valid auto liability insurance
