About the Role

Key Responsibilities
  • Practice independently in the community, delivering high-quality care to patients with diverse needs.
  • Develop and implement individualized care plans, constantly evaluating and adjusting to meet evolving patient needs.
  • Collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of one year nursing experience in a medical/surgical environment.
  • Valid driver's license or NYS Non-Driver photo ID card may be required.
