Position Overview

The Oncology Care Navigator (OCN) is a registered nurse with specialized knowledge in oncology who provides personalized support to patients, families, and caregivers to navigate the complexities of the healthcare system. Utilizing the nursing process, the OCN offers education and resources to empower informed decision-making and ensure timely access to high-quality health and psychosocial services throughout the cancer care continuum. This role is essential in advancing organizational strategies across the cancer care spectrum, requiring the OCN to develop expertise in areas such as community needs assessment, navigation program development, outreach, and understanding ONN-specific outcome metrics. Additionally, cancer survivorship planning is increasingly integral to the responsibilities of the navigator.

Key Responsibilities

1. Collaboration with Healthcare Providers
  • Facilitate the care of patients with a history or potential diagnosis of cancer in accordance with established guidelines.
  • Assist patients and their families in overcoming barriers within the healthcare system.
  • Coordinate additional services, including nutrition, home health, durable medical equipment, and hospice care.
  • Oversee cancer survivorship planning initiatives.
2. Patient and Family Education
  • Deliver education and resources to support informed decision-making and timely access to comprehensive health and psychosocial care throughout the cancer care continuum.
3. Effective Communication
  • Ensure exceptional communication and customer service with both internal and external stakeholders.
  • Provide relevant updates and quarterly patient service data to management.
  • Collaborate with community navigators to assist with resources, education, and planning.
  • Engage in committee participation to fulfill accreditation requirements.
4. Access to Clinical Trials
  • Work with healthcare providers to identify potential clinical trials.
  • Participate in the screening and consenting process for clinical trial candidates.
  • Assist in the initiation of trials, including regimen requests and requirements for blinded studies as needed.
5. Team Collaboration
  • Adopt a team-oriented approach to maintain a comprehensive understanding of program requirements.
  • Assess patient needs for resources and support lay navigators.
  • Participate in outreach initiatives.
  • Coordinate Specialty Infusion Services with lay navigators and staff training.
  • Provide education on advanced care planning and documentation in collaboration with lay navigators.
  • Work with marketing teams to ensure up-to-date materials for providers, patients, and families.
  • Perform additional duties and projects as assigned.
Qualifications

Education

Minimum of an Associate degree in Nursing; a Bachelor of Science in Nursing is preferred.

Experience
  • At least five (5) years of experience in oncology nursing.
  • Experience with clinical trials is preferred.

  • Self-Management: Ability to manage time effectively, prioritize conflicting demands, and pursue professional development. Self-motivated and capable of working independently with minimal supervision, including in a remote environment.
Technical Skills

Proficient in MS Office applications, including Word, Excel, PowerPoint, and Outlook.

Certifications/Licenses:
  • Registered Nurse license required.
  • OCN certification required.
  • Valid Driver's license.
  • Current BLS certification.
  • ONN certification preferred.
  • Public Notary certification.
  • Compliance with the company's Driver Safety Operations and Motor Vehicle Records Check Policy is required.
