Responsibilities- Provide direct nursing care to patients, including assessment, planning, implementation, and evaluation of care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet each patient's needs.
- Conduct thorough assessments of patients, identifying their strengths, weaknesses, and areas for improvement.
- Develop and implement effective care plans that address the physical, emotional, and social needs of patients.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- BLS certification required.
- Nonviolent Crisis Intervention Certification required within 90 days of beginning of employment.
- Certified Registered Rehab Nurse (CRRN) preferred.
- Currently licensed as a Registered Nurse in the state of New Hampshire or appropriate compact licensure, in accordance with law and regulation.
- Associate Degree from an accredited school of nursing required. BSN preferred.
