Job Summary

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans to meet patients' needs and goals
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related field
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
