Occupational Therapist

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients manage chronic conditions and recover from illness or injury.

• Assess and develop individualized treatment plans to improve patients' daily living and work-related skills.
• Administer and interpret diagnostic tests to evaluate patients' level of function and develop effective treatment strategies.
• Collaborate with healthcare teams to develop and implement comprehensive care plans.
• Provide therapeutic treatment and instruction to patients to improve strength, coordination, and range of motion.
• Review and expand treatment plans to ensure patients' goals are met and progress is tracked.
• Consult with other healthcare professionals to develop vocationally oriented plans and coordinate care.
• Communicate patient status and progress to healthcare teams, patients, and families.
• Participate in care coordination and discharge planning activities to ensure seamless transitions.

• Master's degree from an accredited Occupational Therapy program.
• Minimum six months of occupational therapy experience, with home health experience preferred.
• Current and unrestricted OT licensure.
• Current CPR certification.
• Good organizational and communication skills.
• A valid driver's license, auto insurance, and reliable transportation.
