Social Worker Job Summary

We are seeking an experienced Social Worker to join our interdisciplinary team at BoldAge PACE.

As a key member of our team, you will provide psychosocial assessments, individual, family, and group counseling, and case management services to our participants.

Your expertise will help us maintain accurate records, prepare clear reports, and communicate effectively with our team members and community agencies.

Key Responsibilities:
  • Conduct thorough social work assessments to identify participant needs and develop personalized plans
  • Provide individual, family, and group counseling to address mental health concerns, promote independence, and foster healthy relationships
  • Collaborate with our interdisciplinary team to ensure comprehensive care and coordinate services
  • Maintain accurate records and prepare clear reports to facilitate communication and decision-making
  • Develop and maintain relationships with community agencies and resources to support participant well-being
Requirements:
  • Master's degree in Social Work from an accredited institution
  • Current driver's license and proof of auto insurance
  • Certification or licensure as a Social Worker in New Jersey (or willingness to obtain)
  • Minimum 2 years of experience working on an interdisciplinary team in a healthcare setting
  • Experience working with frail or elderly populations preferred
