About the Role

As a Medical Social Worker at Oak Street Health, you will work closely with our interdisciplinary care team to identify the needs of our patients and develop personalized care plans. Your primary responsibility will be to provide timely and effective interventions that address the social determinants of health and support patient self-management.

Key Responsibilities

  • Conduct thorough assessments of patients' social and emotional needs, identifying areas where we can provide additional support
  • Develop and implement individualized care plans that address the physical, emotional, and social needs of our patients
  • Collaborate with the care team to ensure seamless communication and coordination of patient care
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
  • Connect patients with community resources and services to help them access the care and support they need
  • Maintain accurate and up-to-date documentation of patient interactions and outcomes

Requirements

  • Bachelor's or Master's degree in Social Work (MSW) from an accredited institution
  • Minimum 1 year of experience in medical social work or a related field
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare providers
  • Knowledge of community resources and services, including non-profit organizations and government agencies
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
