We are seeking a dedicated and compassionate Registered Nurse to join our team of skilled home healthcare providers at Pinnacle Home Care Inc. As a Baylor Registered Nurse, you will play a crucial role in delivering high-quality nursing care to individuals with various healthcare needs in the comfort of their homes.
Job Summary:As a Registered Nurse with Pinnacle Home Care, you will be responsible for assessing patients, developing and implementing care plans, administering medications, and collaborating with a multidisciplinary healthcare team to ensure the well-being of our clients. You will work a Baylor schedule (4 days, preferably Thursday-Sunday, or Friday-Monday) and have a strong commitment to providing quality care to our patients.
Key Responsibilities:- Conduct initial assessments of patients and families to determine home care needs.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Administer medications and treatments as prescribed by physicians.
- Provide direct patient care, including wound care, vital sign monitoring, and medication management.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
- Participate in on-call duties as defined by the on-call policy.
- Current licensure in the state and CPR certification.
- Graduate of an accredited school of nursing with one to two years of recent acute care experience in an institutional setting.
- Bachelor's degree, with one year of home health care experience preferred.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
