Job Summary:
As a RN-Case Manager at Baptist Memorial Health Care, you will play a critical role in managing the care for a specific population, ensuring the safe transition of patients throughout the continuum of care. Your expertise will be utilized to facilitate the appropriate utilization of resources, service delivery, and compliance with federal and other payer requirements.
Key Responsibilities:
- Early assessment of transitional needs identified during hospitalization, illness, and/or life situation
- Utilization Review
- Discharge planning
- Readmission Reduction Participation
- Payer Communication and denial reduction
Requirements:
- BSN or MSN preferred
- RN with 3 years of clinical experience and Case Management experience in a hospital or payer setting preferred
- RN with at least one (1) year of clinical experience minimum
Preferred Skills:
- Critical thinking skills
- Communication, organization, interpersonal, and computer skills
- Knowledge of payer requirements, problem-solving, and governmental regulations
