LPN Home Health Care Specialist

Job Summary:
Baptist Health is seeking a skilled LPN to provide high-quality nursing care in a community setting. As a Home Health LPN, you will work under the direction of a Registered Nurse to deliver patient-centered care, promoting health, rehabilitation, and wellness.
Key Responsibilities:
- Deliver skilled nursing care in accordance with Agency policies and procedures.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct thorough assessments, identify patient needs, and provide appropriate interventions.
- Monitor patient progress, adjust care plans as needed, and communicate effectively with the healthcare team.
- Provide education and support to patients and their families to promote health and wellness.
Requirements:
- Associate degree or equivalent training in nursing.
- Current LPN licensure.
- BLS Healthcare Provider certification.
- At least two years of clinical nursing experience.
- Home care experience preferred.
