Job Summary

Citizens Medically Frail Intermediate Care Facility (ICF) is seeking a Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing enhanced medical support to our residents, who require 24-hour nursing services and habilitative services.

Responsibilities
  • Administer medication and medical treatments, and monitor the health status of our residents.
  • Coordinate the delivery of primary and specialized medical services under the direction of the RN, and serve as a medical services advocate for each individual.
  • Practice and promote the agency's mission statement in every interaction.
  • Provide ongoing support to our residents in becoming active participants in every aspect of their lives and ensure a high quality of life is maintained.
Requirements
  • Must be a NYS licensed practical nurse.
  • CPR, fire safety training, and medication knowledge is required.
Work Environment
