About the Role

Rising Ground is seeking a highly skilled Case Manager to join our Unaccompanied Children Program. As a Case Manager, you will be responsible for managing a caseload of 8 children who will be reunified with their sponsor in the US, safely and expeditiously.

Key Responsibilities
  • Manage a caseload of 8 children and provide case management services, including developing and maintaining contact with youth and their service providers for medical, social, educational, and other related service needs.
  • Perform discharge and release functions to ensure that each youth has a safe, appropriate, and prompt release plan.
  • Follow up with legal service providers regarding the timely submission of any documentation or correspondence needed for the legal relief process.
  • Document client contacts and other case required paperwork in an accurate and timely manner in accordance with regulations.
  • Ensure all ORR, OCFS, and Education mandates and requirements are met.
  • Work collaboratively with the Clinical Team and other program staff across the department and the organization.
  • Participate and attend all required program-related meetings and conferences.
  • Maintain knowledge of issues affecting UC.
  • Participate in the implementation of PBIS to promote and reinforce a positive social culture with all persons supported and staff at POH.
  • Support and participate in the group interaction process.
  • Ensure that home visits to foster parents occur and that all pertinent information is relayed appropriately to foster parents.
  • Perform other duties as assigned.
Requirements
  • Bachelor's in a Human Services field.
  • Knowledge of child welfare and immigration-related legislation.
  • Excellent verbal, and written communication and organizational skills are essential.
  • Proficient in Microsoft Office suite, knowledge of Connections.
  • Bilingual (English/Spanish) a must.
