As a Practical Nursing Specialist at ConnextCare, you will play a crucial role in providing exceptional patient care and support within our healthcare team.

Key Responsibilities:

  • Assist patients in transitioning to examination areas while ensuring accurate documentation in compliance with all relevant regulations.
  • Facilitate a smooth patient flow throughout the day, adhering to the scheduled appointments.
  • Provide educational resources to patients and address any inquiries they may have.
  • Implement medical instructions as directed by licensed healthcare providers, including conducting tests, managing treatments, and coordinating referrals.
  • Accurately document patient visits in the electronic health record, ensuring all services rendered are properly charged.
  • Engage in case finding, health education, counseling, and restorative care under the supervision of a registered nurse or licensed healthcare professional.
  • Maintain cleanliness and organization of medical supplies and ensure proper sanitation after each patient interaction.
  • Participate in quality improvement initiatives, training workshops, and continuing education as requested by management.
  • Follow up on laboratory results and specialist referrals as directed by healthcare providers.
  • Adhere to all safety regulations and organizational policies to ensure a safe and compliant work environment.
  • Contribute to Patient Centered Medical Home initiatives as an integral member of the Clinical Care Team.
  • Perform additional duties as assigned by management or supervisory staff.

Qualifications:

To be considered for this role, candidates must possess:

  • A diploma from an accredited nursing program and a valid New York State practical nursing license.
  • A willingness to accept guidance from healthcare providers and supervisors.
  • The ability to maintain composure and compassion in high-pressure situations.
  • A professional demeanor and respect for confidentiality in all interactions.

Compensation:

Hourly wage commensurate with experience.
