The primary role of the Medical Social Worker II is to provide comprehensive social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. As an integral member of the healthcare team, the Medical Social Worker II delivers age-appropriate social work care to patients and their caregivers, providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.
Key Responsibilities:- Assessment and Treatment Planning: Conduct psychosocial assessments, develop and implement individual treatment plans, and provide counseling for disease acceptance and understanding.
- Case Management: Coordinate exchange of information between healthcare providers, families, and caregivers, and determine application of benefits to specific patient situations.
- Community Resource Referral: Provide information and referral to community resources as requested, and collaborate with internal and external resources to meet mutually agreed upon goals and objectives.
- Communication and Collaboration: Work with referral sources to clarify and complete required clinical and psychosocial information, and participate in Utilization Management/Quality Assurance activities.
- Documentation and Record-Keeping: Maintain accurate and timely documentation, records, and data collections.
- Education: Master's in Social Work accredited by the Council of Social Work Education.
- Experience: 2-4 years social work experience within the last five (5) years.
- Skills: Solid psychosocial assessment skills, excellent oral/telephone communication skills, and ability to work independently as a collaborative team member.
- Post-MSW Experience: At least one (1) year post MSW experience in a healthcare setting, preferably with LCSW certification.
