Job Description

6th Avenue Senior Living seeks a dedicated Part-Time Licensed Practical Nurse (LPN) to deliver exceptional care to elderly residents in our Assisted Living and Memory Care environments.

About the Role

This position requires an individual who is committed to providing high-quality care, demonstrating reliability, and possessing strong customer service skills. The selected candidate will be responsible for monitoring the health and well-being of our residents, managing diabetic conditions, communicating with healthcare providers, and contributing to care conferences.

Responsibilities
  • Monitor and maintain the physical and emotional well-being of our residents.
  • Administer insulin and perform blood sugar checks for diabetic residents.
  • Communicate effectively with outside healthcare providers regarding resident status.
  • Maintain open communication with family members, powers of attorney, and legal guardians.
  • Contribute clinical information to care conferences and planning processes.
  • Oversee and monitor the care provided by caregivers and med techs.
Requirements and Qualifications
  • CURRENT ACTIVE LPN license required.
  • Knowledge of nursing practices, particularly in geriatric care, and state boarding home regulations.
  • Experience with Therapeutic Options and Hand in Hand training is advantageous.
  • Exceptional interpersonal and communication skills.
  • Ability to work efficiently in a fast-paced environment.
  • Dependable, team-oriented, caring, and able to foster dignity and respect.
  • Requires a positive attitude and high energy level.
  • Strong attention to detail.
Compensation and Benefits

The estimated salary range for this role is $65,000 - $80,000 per year, depending on experience and qualifications. This position offers competitive benefits, including comprehensive health insurance, paid time off, and opportunities for professional growth and development.
