About the Role

We are seeking a highly skilled Placement Case Manager to join our team at Children's Home Network. As a key member of our placement services department, you will play a critical role in ensuring the well-being and safety of children in our care.

Job Description

This position involves assessing the needs of children in care and identifying suitable foster families or group homes. You will work closely with various stakeholders, including Child Protective Investigators and case management organizations, to ensure that children receive the best possible care.

Key Responsibilities:
  • Assess the needs of children in care and gather relevant information about their medical, dental, behavioral, educational, mental health, emotional health, trauma history, likes, dislikes, strengths, and needs.
  • Use our designated data system to identify potential placements and determine the most suitable options for each child, considering factors such as siblings, school districts, removal areas, and individual needs.
  • Document all efforts in matching children with foster families and ensure that all relevant facets are considered when making decisions.
  • Complete Match reports within 24 hours of placement and maintain open communication with licensing teams, agencies, and residential providers.
  • Participate in on-call rotations to respond to immediate placement needs and collaborate with transportation units to ensure timely coordination of transportation.
  • Attend QPI-related events, daily meetings, and staffings to assess children in need of placement and begin pre-planning for future steps.
  • Maintain accurate records and provide post-placement support to help stabilize placements.
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in social work or a related field from an accredited institution.
  • At least one year of experience working with special needs children in a child welfare setting.
  • Relevant case management experience and knowledge of foster care practices are highly desirable.
  • CWCM or CWCL certification is required.
