Nurse Practitioner

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) to join our team. As a RN, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Responsibilities:
- Make home visits to clients in designated geographic territories after business hours and during the weekend days and evenings.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
- Monitor clients' conditions; report changes to Clinical or Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
Requirements:
- A current license as a Registered Nurse in VT/NH.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred, but not required.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
