  • Providing direct patient care in accordance with established protocols and standards
  • Assessing patient needs and developing individualized care plans
  • Collaborating with interdisciplinary teams to ensure seamless care transitions
Requirements

To be successful in this role, you must possess:

  • A valid license to practice as a practical nurse or vocational nurse in the state of operation
  • One year of experience as a Licensed Practical Nurse (LPN), preferred
  • Valid CPR certification, preferred
  • The ability to meet regulatory requirements and maintain required certifications
