Job Summary

We are seeking an experienced Advanced Practice Nurse (APN) to join our Stroke Program as a Coordinator. The successful candidate will be responsible for implementing and ensuring optimal inpatient care and outcomes for stroke patients, in accordance with evidence-based standards and guidelines.

Key Responsibilities:
  • Develop and implement protocols for stroke care, ensuring compliance with state, national, and Joint Commission Comprehensive Stroke Center requirements.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Collect and analyze data to identify areas for quality improvement and implement strategies to enhance patient outcomes.
  • Develop and maintain relationships with healthcare providers, payers, and community organizations to promote stroke awareness and education.
  • Stay up-to-date with the latest research and advancements in stroke care, incorporating best practices into program development and implementation.
Requirements:
  • Current Colorado Clinical Nurse Specialist License or Nurse Practitioner License.
  • Current BLS and ACLS Certification through AHA or ARC.
  • Prescriptive Authority granted by the Colorado Board of Nursing and DEA Certificate, if prescribing pharmacologic agents and/or writing medication orders.
  • Graduate of an Accredited School of Nursing and completion of an accredited program for advanced practice nurses, with a Master's Degree required.
  • Certification by National accrediting body in his/hers specialty area, preferred.
  • 1-2 Years Advanced Practice Nurse (APN) provider experience, preferred.
  • 5-7 Years related previous experience as a nurse or advanced practice nurse, preferred.
