Job Summary

Key Responsibilities
  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure ongoing care and coordination.
  • Deliver infusion nursing care in the home or clinic, working closely with patients and their families to achieve optimal outcomes.
  • Develop and implement individualized care plans, taking into account patients' unique needs and circumstances.
  • Communicate effectively with healthcare teams, patients, and families to ensure seamless care transitions and coordination.
Requirements
  • Unencumbered Registered Nurse License in the state(s) of hire and wherever care is delivered.
  • 1 year of experience in acute hospital care, infusion, or home health.
