Role Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health is accountable for assessing and evaluating patient care requirements concerning mobility, endurance, pain, and other health conditions. Based on these evaluations, the Physical Therapist collaborates to create a treatment plan, implements interventions to enhance the patient's well-being, and monitors their progress.
- Assess the patient's functional capabilities and physical therapy requirements, assisting the physician in formulating the therapy care plan.
- Monitor and report the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
- Administer treatments to alleviate pain, restore function, and maintain optimal functioning in accordance with established standards of practice.
- Educate and inform all members of the healthcare team, the patient, and their family/caregivers about the care plan, functional limitations, and progress towards goals.
Licensure Requirements
- Active Physical Therapy license in the state of practice
- Current CPR certification
- Possession of a valid driver's license and vehicle insurance, along with reliable transportation
