Job Summary

This position provides medical care and treatment services with a focus on chronic disease management and promotion of health and palliative care for patients who are not best served in a clinic setting. The physician will work collaboratively with other medical and mental health professionals to provide comprehensive care to patients in their homes.

Responsibilities
  • Provide medical consultation and support to other practitioners within the organization.
  • Diagnose and treat acute and chronic medical conditions, including conducting physical exams, ordering and interpreting diagnostic tests, and devising a course of treatment based on patient preferences and goals.
  • Develop and implement proactive patient-centered care plans with the assistance of other disciplines within the care team.
  • Provide compassionate anticipatory guidance through decline due to chronic disease and provide high-quality palliative care focused on the goals of the patient and family.
  • Prescribe and dispense medications and drugs to suitable patients, initiate and/or continue medical regimes for new patients, and chart treatment records and monitor results.
  • Develop and maintain working relationships with lay and professional caregivers, including care facilities, to ensure patient experience excellent continuity of care and orders are implemented appropriately.
  • Educate patients and families on goal-based, appropriate preventative care, chronic care or urgent care needs, as well as the use of prescribed medications and treatment.
  • Participate in collaborative treatment with other medical and mental health professionals and technical staff.
  • Participate in staff and professional conferences, consult on and assist in the development of in-service training and orientation activities in areas of treatment and care of the patient population.
Requirements
  • Licensed physician (MD or DO) in the State of Oregon.
  • Completion of internship and residency in Internal Medicine or Family Medicine.
  • Certification by the American Board of Medical Specialties.
  • Drug Enforcement Administration (DEA) license.
  • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
  • CPR certification at hire or within 6 months in position.
Preferred Qualifications
  • Geriatric and/or palliative care certified.
  • Experience in medical home visits.
Working Conditions

This position requires travel outside of the workplace at least weekly, and the employee's personal vehicle may be used. Driving infractions will be monitored in accordance with organizational policy.
