SAN ANTONIO METROPOLITAN MINISTRY Inc is seeking a qualified Permanent Supportive Housing Case Manager II to join our team. The successful candidate will have a bachelor's degree in social work, human services, or a related field, and at least 2 years of case management experience.

The Permanent Supportive Housing Case Manager II will be responsible for providing comprehensive and intensive case management services to chronically homeless individuals residing in the community. This includes building trusting relationships with participants, facilitating client self-sufficiency and housing retention, and utilizing evidence-based practices such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, Housing First, and Person-Centered Planning.

Key Responsibilities:

  1. Provide onsite case management services that facilitate client self-sufficiency and housing retention.
  2. Build positive and trusting relationships with diverse populations, particularly participants experiencing trauma, homelessness, physical and/or mental health disabilities, and substance use challenges.
  3. Use evidence-based practices in service delivery, such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, Housing First, and Person-Centered Planning.
  4. Maintain on-going case load of 12-13 individuals participating in program by meeting with each participant on a weekly basis.
  5. Assist individuals in developing an Individualized Service Plan and actively involve participants in the design and delivery of supportive services.
  6. Conduct crisis assessment and provide appropriate interventions.
  7. Maintain necessary documentation, reports, and statistics in the Homeless Management Information System (HMIS) database.

Requirements:

  • Bachelor's degree in social work, human services, or a related field.
  • At least 2 years of case management experience.
  • Previous experience working with at-risk populations and crisis intervention is preferred.
  • Bilingual (English/Spanish) is required.
  • A valid Texas Driver's License and liability insurance is also needed.

Estimated Salary: $55,000 - $65,000 per year, depending on experience.
