Job Title: Advanced Care Registered Nurse

Job Summary:

DispatchHealth is seeking a highly skilled and compassionate Advanced Care Registered Nurse to join our team. As an Advanced Care Registered Nurse, you will play a pivotal role in delivering high-quality, patient-centered care in the comfort of patients' homes. You will work closely with our multidisciplinary team to provide comprehensive care, educate patients and their families, and promote health and wellness.

Key Responsibilities:

  • Provide direct nursing care to patients in alternative care settings, including lab draws, IV insertion, and medication administration.
  • Engage patients in their healthcare journey, provide meaningful education tailored to the patient's needs, and establish meaningful connections and trusting relationships with each patient.
  • Implement multidisciplinary plans of care for patients in Advanced Care program services, working closely with the Advanced Care leadership team to support and promote care redesign.
  • Oversee the planning, scheduling, and implementation of day-to-day clinical activities for patients, ensuring timely and effective care delivery.
  • Act as a central point of contact for all navigated patients, interacting with medical, nursing, and ancillary services to ensure seamless care coordination.
  • Collaborate with medical providers, patient care staff, and skilled services to plan and implement patient and education plans, ensuring educational goals are on track and patients are actively engaged in their care.
  • Monitor patients daily for compliance with the plan of care, tracking order completion and follow-up, and engaging with patients throughout the episode of care.
  • Assist patients and care partners with referrals to community resources and care coordination, addressing social determinants of health as needed.
  • Monitor patient progress to goals in the post-30-day period, designing or participating in the creation of Advanced Care materials, including education materials, assessment plans, and interdisciplinary plans of care.

Requirements:

  • Certification/Licensure in the State of Virginia as a Licensed Registered Nurse or licensure from a state participating in the multistate privilege to practice compact with Virginia.
  • Minimum of 2 years acute care nursing experience.
  • BLS and ACLS Certified.
  • BSN, preferred.
