Job Summary:
Soleo Health Inc is seeking a skilled Per Diem Infusion Nurse to provide primary patient care for assigned patients in the San Francisco, CA area.
Responsibilities:
- Provide direct care for assigned patients, including initiation and maintenance of communications with care team members.
- Evaluate and educate patients/care partners on safely administering home infusion therapy.
- Evaluate the home environment and identify interventions required.
- Develop and maintain caseload scheduling and communications.
- Document patient visits and discharge summaries.
Requirements:
- Excellent IV skills.
- RN degree, BSN preferred.
- Minimum 3 years nursing experience with background in infusion therapy.
- Current nursing license for applicable state.
- High school diploma (or equivalent) required.
- Experience in ICU and/or Cardiac Care preferred.
- Excellent customer service skills.
- Proficient with Microsoft Excel, Word, and Outlook.
