Position Summary

The Practical Nursing Specialist is responsible for planning and overseeing patient care tailored to individual needs. This role involves interviewing patients to gather their medical history and current health status, as well as monitoring vital signs such as pulse, blood pressure, temperature, and respiration. The position requires providing essential care to patients on a routine basis.

Key Responsibilities
  • Deliver personalized 1:1 care for clients, adhering to their care plans while respecting their dignity and independence.
  • Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
  • Administer medications, assist with feeding, provide oxygen therapy, and perform ostomy care as outlined in the client's care plan.
  • Collaborate with other healthcare professionals, including Physical, Occupational, and Speech Therapists, to complete established routines.
  • Maintain infection control protocols, report potential communicable diseases, and ensure adherence to infection control practices.
  • Document all nursing actions accurately and communicate with family members, case managers, and physicians regarding patient progress and changes in medical conditions.
  • Order necessary supplies and medications to maintain adequate inventory levels.
  • Ensure that all patient needs are met in accordance with physician directives.
  • Monitor incidents involving clients, ensuring timely and accurate completion of incident reports, and inform case managers accordingly.
  • Ensure proper maintenance of medical equipment and report any malfunctions for repair.
  • Perform additional duties as assigned.
Qualifications
  • Completion of post-secondary education required for licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Understanding of individual service plans, including client rights and health requirements.
  • Current and valid license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing.
  • CPR certification is mandatory.
  • Strong interpersonal skills, including customer service, patience, empathy, and compassion.
  • Thorough knowledge of patient care policies and regulations.
  • Familiarity with symptoms and indicators of improvement for various medical conditions.
  • Willingness to travel between facilities as needed.
