Key Responsibilities- Conduct comprehensive patient assessments, including physical examinations, and collect clinical data to inform treatment decisions.
- Develop and implement individualized treatment plans, taking into account patient needs, medical history, and current health status.
- Collaborate with healthcare providers to ensure seamless care coordination and communication.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures as needed.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Master's degree in Nursing from an accredited program.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of New York.
