Essential Functions:
- Prepare and administer medications to residents in accordance with established policies and procedures.
- Conduct narcotic counts at the start and end of each shift.
- Order medications from the pharmacy as per community guidelines.
- Review Medication Administration Records (MAR) for errors in compliance with community policy.
- Communicate with residents' families regarding medication needs as necessary.
- Manage expired or discontinued medications and prepare them for destruction.
- Administer treatments, monitor vital signs, and assist with resident weight assessments.
- Document changes in residents' conditions and unusual occurrences accurately.
- Ensure all lab work, physician orders, and medication orders are processed and filed according to community standards.
- Assist with scheduling doctor appointments as required.
- Support new resident admissions following community protocols.
- Help residents with personal care tasks, including grooming, dressing, and toileting.
- Facilitate activities of daily living as outlined in individual care plans.
- Assist with light cleaning and maintenance tasks as needed.
- Respond promptly to incidents while maintaining residents' dignity.
- Provide support to residents who have fallen, ensuring their safety and comfort.
- Accompany residents to and from dining areas as required.
- Manage laundry tasks for residents as needed.
- Assist in meal service and beverage distribution.
- Respond to call lights with patience and empathy, adhering to company protocols.
- Engage with residents, offering reassurance and addressing their concerns.
- Conduct rounds to ensure resident safety and provide assistance as necessary.
- Communicate changes in residents' conditions to Health Services Director, physicians, and families as needed.
- Participate in required meetings, training sessions, and in-services.
- Maintain confidentiality of residents' personal information and uphold their rights.
- Foster positive communication with residents, families, visitors, and staff.
- Provide tours to prospective residents and families, showcasing how their needs can be met.
- Exhibit a professional demeanor that promotes a nurturing environment for residents, families, and guests.
- Ensure excellent customer service through open communication and responsiveness to the needs of residents, families, and staff.
- Coordinate with culinary services to ensure adequate meal service coverage.
- Embody a servant-hearted attitude in alignment with Lifestream's mission.
Qualifications:
- Strong interpersonal skills for effective communication with residents, staff, and families.
- Genuine compassion for the elderly and disabled.
- Knowledge of hazardous materials and safety guidelines.
- Ability to work independently and prioritize tasks in emergency situations.
- Capability to handle stressful interactions with residents, families, and staff.
- Proficient in safe medication administration practices.
Education and Experience:
A high school diploma or equivalent is preferred. Certification in medication administration and CPR/First Aid is required per state regulations.
Physical Requirements:
This position requires moderate physical activity, including lifting objects up to 50 pounds occasionally and up to 20 pounds frequently. Candidates must be able to stoop, bend, and reach, and assist in lifting residents over 100 pounds with support.
